RENEWAL OF PASSPORT

The P.F.82 or P.F.136 - Application for the renewal of a passport - should be used to apply for the renewal of a Mauritius passport

Each applicant must submit:

Present Mauritius Passport
Two passport size photographs endorsed on the reverse side by two persons, who have known you personally for at least two years and who are either Members of the Legislative Assembly, Medical Practitioners, Ministers of Religion, Barristers-at-Law, Notaries Public, Bank Managers, Police Officers not below the rank of Assistant Superintendent or Civil Servants not below the rank of
Higher Executive Officer, with the words: "We certify that this is a true likeness of Mr. or Mrs or Miss ...." and add their signature.
The following written and signed declaration in paragraph 8 (eight) of page 4 of P.F.82 I certify that I have not renounced the citizenship of Mauritius. I certify that I have not obtained a foreign nationality before the age of twenty-one
PASSPORT APPLICATION FORM

You can download the Acrobat Reader version that is suitable to your system from the Adobe Acrobat Web Site. If you already have Adobe Acrobat Reader, Version 3.0 or higher installed, you can click here to view or print the passport application now or print the new passport application form.

NOTE: We accept money orders and bank drafts. Below is a table of fees for passport-related services is annexed. (RATES BELOW ARE IN US DOLLAR)

1.	Issue a new passport
	(i) Minor (under 16y/o) - 5 years validity	$35.00
	(ii) Young Person (16-18y/o) - 10 years validity	$50.00
	(iii) Adult (18 to 55y/o) - 10 years validity	$50.00
	(iv) Senior Citizen (55 & above) - 10 years validity	$35.00
2.	Renewal of a passport	$40.00
3.	Endorsement of a passport	$15.00
4.	Inclusion of a child's name in a passport	$15.00
5.	Issue of a certificate of identity or other travel document in lieu of passport	$25.00
6.	Where passport, during its period of validity -
	a. is declared lost or damaged; or -
	c. is found to be unusable due to tampering -
	First issue of a new passport -	$180.00
	Second issue of a new passport -	$300.00
	Third issue of a new passport -	$470.00
	Each subsequent issue of a new passport an additional fee of -	$180.00
